Southern Utah Hospice complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, or sex. Notice of Nondiscrimination
Southern Utah Hospice does not exclude people or treat them less favorably because of race, color, national origin, age, disability, or sex. We provide, free of charge and in a timely manner:
- Reasonable modifications and appropriate auxiliary aids and services for people with disabilities to communicate effectively with us — qualified sign-language interpreters, and written information in other formats such as large print, audio, and accessible electronic formats.
- Language assistance services for people whose primary language is not English — qualified interpreters and information written in other languages.
If you need these services, call 435-635-1001. If you believe Southern Utah Hospice has failed to provide these services or discriminated in another way on the basis of race, color, national origin, age, disability, or sex, you can file a grievance by phone at 435-635-1001, or by mail to 690 N. Industrial Rd #3, St. George, UT 84770. If you need help filing a grievance, we will help you.

Notice of Availability of Language Assistance Services and Auxiliary Aids and Services
ATTENTION: If you speak a language other than English, language assistance services are available to you free of charge. Appropriate auxiliary aids and services to provide information in accessible formats are also available free of charge. Call 435-635-1001.

Why this page exists
Section 1557 of the Affordable Care Act (45 CFR Part 92) requires every provider that accepts Medicare or Medicaid to publish these two notices.
